Some suggestions to make this work better for me:

In addition to the fields you already provide you might consider adding the following 3 fields to each reminder:

1. Occasion (popup), Birthday, Anniversery, etc

2. Relationship (popup), Son, Daughter, Mother, Father, Wife, Sweethart, etc

3. Notes, free typing

When you enter a reminder date such a birthday that is actually on 6/22 you should get 1st reminder on 6/15 and 2nd reminder on 6/19

In other words the date should remain actual and reminders should come before the actual date as illustrated above
