To the public, to the media (at least with cameras on or tape rolling) the candidate is always referred to by name and title if he has one. (I've said he throughout these comments for ease of use, but it applies to both gender candidates)

Staffers regularly call the candidate the boss or other nicknames (not always polite) among themselves, but this should never be said publicly. Doing so usually puts the candidate in a bad light, and diminishes the appearance of an egalitarian team.

It can look bad and as if the candidate is oppressive. Plus it can actually diminish respect for the candidate if their helpers call them a nickname in public.
