A great intimidating howling cartoon face.

I couldn't resist buying it for the cover alone.

The cover shows a red face in a look of astonishment OR fear, looking off to the side with a wide open mouth, and you can see the teeth, tongue, etc. I've read in a book about the band that 'King Crimson" is a reference to the devil, and the lyrics of the title track seem to bear that out. It's also somewhat reminiscent of the entrance to The Inferno, I guess. There's a slightly similar and slightly earlier album cover by a very obscure Ohio group called Ant Trip Ceremony too, except that the group are drawn inside the mouth.
