I personally would like to institute a 90-day grace period for mixing up names. It only makes sense that as time goes by and you date more and more people, there's a chance you might slip up. Many girls I know would end things immediately, feeling utterly disgusted and turned off at the very idea that a man could mistake her for a former lover! I was very relieved that between '01 and '03, nearly every girl I met was named Jessica. I confused their names with no ramifications whatsoever. Since I moved to San Francisco there's been Urszula, Brooke, Rwand, and Maha. Where and why do they come up with these names.
