Second in every instance I've seen, the "reality" is entirely a contrived situation, such as stranding a telegenic selection of focus group tested contestants on a deserted island with nothing more than their wits and a nearby TV crew living in air conditioned trailers with craft servies providing food, or following celebrities around while they pretend that they don't know there is a camera and lights on them.
