Frequently hear it usually slightly paraphrased.

It comes from Hamlet, and is spoken by Hamlet's mother, Queen Gertrude, about the queen in the "play within a play" that Hamlet stages to trap his mother and uncle. The irony here is that Gertrude is commenting on the fictional mirror of herself.

This is one of those quotes which remains in old English for popular use. I've seen it used in Internet forum debates on numerous occasions.
